If an Explanation of Benefits (EOB) is sent to my family’s address, what will it say?

An Explanation of Benefits will usually contain information like:

  • Your name and address
  • The date of your visit
  • A description or procedure code for any services or procedures you received
  • The name of your provider
  • How much was billed to your health insurance plan
  • How much your insurance paid for your services or procedures
  • Codes for any adjustments or denials
  • Any remaining balance that will be billed to you

The description of services/ procedures you received could be named or detailed. If this kind of personal health information is something you do not want your parent, guardian, spouse, or other family member(s) to see, you need to submit a request for confidential communication to your insurance company BEFORE you receive any services at the Student Health Center. It could take your insurance company anywhere from a week to a month to process your request, so you need to submit the request as early as possible, and call your insurance company to confirm whether or not your request has been processed yet, before coming to the SHC for services.

If you receive healthcare services before making a request for confidential communication, or before the request has been processed, your Explanation of Benefits (EOB) containing your personal health information will be sent to the policyholder for your health insurance plan.
