Sample Consent Forms

This website has been updated to reflect the Revised Common Rule, implemented on January 21, 2019.

All of these sample forms may be saved as Word documents and edited to reflect the specific details of your research proposal.

Sample 1: Written Consent Form for Adults
Written at a reading grade level of 11.5 per the Flesch-Kincaid Scale.

Sample 2: Written Consent Form for Adults (Currently under development)
Written at a reading grade level of 7.4 per the Flesch-Kincaid Scale.

Sample 3: Written Consent Form for Parents or Legal Guardians of Minors (Currently under development)
Parents, legal guardians, or a legally authorized official must sign consent forms permitting minors to participate in research projects. Depending on the age of the minor, you may wish to have the minor sign an assent document. The form signed by the minor is for informational purposes and to make the minor feel more involved in the study. In most situations, minors are not of legal age to provide informed consent. The assent document cannot substitute for the adult authorization form, but merely supplement it.

Sample 4: Sample Assent Form for Minors (Currently under development)
Written at a reading grade level of 8.2 per the Flesch-Kincaid Scale. Tailor the form to meet the reading level of the children involved.
Parents, legal guardians, or a legally authorized official must sign consent forms permitting minors to participate in research projects. The assent document for minors must be prepared with the same thoroughness as the informed consent document for adults.
