All regular full-time, benefit eligible employees may choose from three healthcare options with Providence Connect, Kaiser HMO and Kaiser Added Choice (POS). The medical plans offer major medical and vision coverage along with alternative care options. Eligible spouses, domestic partners and children can be added to University healthcare plans.
The lowest cost level is basis for the University's contribution in paying employee premiums. If the employee chooses the buy up Added Choice Plan, the employee will pay the difference. Premiums are quoted at monthly rates for each level of coverage.
Employee premium costs are paid through a pre-tax payroll reduction each payday. If employment should terminate while enrolled, employees are guaranteed the right to continue coverage for themselves and/or their dependents on a self-pay basis provided they exercise the COBRA conversion plan option within 60 days of leaving the University.
Medical insurance benefits are effective on the first of the month following the employee’s start date. A University-wide open enrollment period during November provides the opportunity to change health plans and covered dependents effective January 1st. Dependents can be changed outside of open enrollment if a qualifying life event occurs.
Newborns are covered from the moment of birth for the first 31 days of life. In order for coverage to continue beyond this 31-day period, you must submit an enrollment application with a copy of the newborn's birth certificate to Human Resources within 31 days after the child’s birth.

Dental insurance is provided for full-time and benefit-eligible part-time employees and their eligible family members on the first of the month following the employee’s start date. Employee premium costs are paid through a pre-tax payroll reduction each payday. Dental insurance plans are available from Kaiser Permanente or Moda Delta Dental.
